Immunization Requirements beginning with Fall 2016 enrollment*

Welcome to Aurora University.  As you prepare to join the AU community, specific information needs to be on file in the Wellness Center PRIOR to your arrival.  If you will be enrolling in 9 or more credit hours of on-campus class instruction and/or living in a resident hall on-campus you are required to complete our Student Health/Immunization Record form.  The front side provides the Wellness Center  with information about your general health, and the back side provides information about your immunization history required by the State of Illinois College Immunization Code (77 ILL. Adm. Code 694) and Aurora University. 

When completing the immunization history section pay close attention to the detailed immunization requirements listed below, as failure to provide all required information could result in delay with your registration. 

Required Immunizations

  1. Measles (Rubeola):  Two doses
  2. Mumps:  Two doses
  3. Rubella:  Two doses
  4. Tetanus, Diphtheria, Pertussis (DTP, DTaP, Td, Tdap): At least three doses of past Tetanus, Diphtheria and Pertussis containing vaccine are required.  One dose must be Tetanus, Diphtheria acellular Pertussis vaccine (Tdap).  The last dose of vaccine (DTP, DTaP, Td or Tdap) must have been received within 10 years prior to the term of current enrollment. 
  5. Meningococcal:  If 21 years of age or younger, one dose of meningococcal conjugate vaccine on or after 16 years of age is required.   

Special Note:  All records must be in English, clearly legible and include your name and date of birth to be considered admissible.

* Students who will enroll at AU for the first time or will begin a new degree program in Fall 2016 or later are subject to these requirements.  In addition, students who were enrolled prior to Fall 2016 and who have begun a new degree program (e.g. previously earned a Bachelor's degree and will begin a Master's degree; previously earned a Master's in Educational Leadership and will begin a Master's in Educational Technology) will be subject to the Fall 2016 immunization requirements.  In addition, students who were on a leave of absence or previously withdrew from AU and need to reapply in order to resume their studies will be subject to the Fall 2016 immunization requirements.
