Intercollegiate Athletics Participation Forms
The forms located below are intercollegiate athletic participation forms required for athletics participation during the 2008-2009 academic year.
Please note that due to a change in NCAA legislation, all new, first-time student athletes at Aurora University should receive their pre-participation exam no earlier than June 1, 2008 for the 2008-2009 academic year. However, it is strongly recommended that all new and returning students-athletes have their pre-participation completed between June 1, 2008 and August 1, 2008 to avoid any delay with athletics participation.
Prior to ANY athletics participation, the following information must be on file with the Head Athletic trainer - proof of a recent physical exam and proof of insurance. Both of these forms, and a sheet detailing the provisions of our excess athletic accident insurance, are located below. Please download all three forms and return the Student Athlete-Health History form and Emergency Contact/Insurance information form prior to August 1, 2008. Complete all sections; incomplete forms may result in delayed participation in intercollegiate athletics. Upon completing these forms, please mail completed forms to:

1. Student-Athlete Health History Form (PDF file, updated 18 April 2007)
The physical exam should take place between June 1, 2008 and August 1, 2008; all new, first time student athletes at Aurora University should not receive their physical prior to June 1, 2008. This form is two pages and must be returned to AU by August 1, 2008. Please make a copy to keep for your records.
- A physician (MD or DO) must vouch/sign this form. Physicals signed by registered nurses (RN), naprapathic doctors or chiropractors (DC) are not accepted.
- New Aurora University Students: Please complete EITHER the Student-Athlete Health History form or the Certificate of Health and Immunity. The Certificate of Health and Immunity is available from the Wellness Center (www.aurora.edu/wellness/index.htm). On the first page of the form please indicate that you are a student-athlete and a copy will be forwarded to the Athletic Training Staff.
- If you receive a physical and use a form not provided by Aurora University, submit the completed physical form (with physician signature) with the AU Student-Athlete Health History form (only complete sections 1, 2 and 3).

3. Emergency Contact and Insurance Information Form (PDF file - updated 18 April 2007)
Please make a copy to keep for your records and return the original to AU no later than August 1, 2008.
Proof of Insurance MUST be submitted to the Athletic Training Staff prior to ANY athletic participation.
"Proof of Insurance" includes providing Aurora University training staff with a copy of both the front and back of your insurance card. The requirements for waiving student accident insurance and showing proof of insurance are separate. We apologize for any inconvenience. Failure to provide this information will delay intercollegiate athletic participation.
Please note: the excess athletic accident insurance has a $250 deductible. The deductible is applied after all personal insurance has paid on the claim. To submit a claim with the University's excess athletic insurance, you must complete an additional form.
