2009-2010 Financial Aid Forms

The financial-aid process can be very intimidating. To help make your experience a bit smoother, we have created a guide listing step-by-step instructions. Start here, then download the forms below, as needed throughout the process.

All forms are available as Adobe Acrobat files that can be downloaded. Once the forms are downloaded, they should be completed, signed and returned to the Office of Financial Aid (347 S. Gladstone Ave, Aurora, IL 60506). Except for the Stafford and PLUS Promissory Notes, all forms may be faxed to 630-844-6191.

A. General Information and Misc. Forms

B. Verification Forms:

If you were selected for verification for the 2009-2010 academic year, your financial-aid counselor will request the following:

  • A signed copy of your 2008 Federal Income Tax Return and ALL Schedules.
    • Unsigned tax returns will be returned to you for your signature.
    • Please Note: Parent or Spouse Federal taxes may also be requested
    • Forms #8453, #8879, and Letter #1722 will NOT be accepted.
  • It will also be necessary to submit ALL 2008 W-2s used in filing your Federal Income Taxes.
    • W-2 Wage Summaries will only be accepted if they include information from Boxes 12A-12D.

To order a duplicate tax return, please contact the IRS at 1-800-829-1040 and request a line-by-line tax transcript or complete and mail Form 4506-T to the IRS .

D. Student Loans

  3. Federal Stafford Loan Master Promissory Note (MPN) Please complete, make a personal copy and mail to the Financial Aid Office (we are unable to accept faxes of this document). Form is invalid if white-out is used. - (.pdf)

E. Loan Counseling Sessions

  1. Stafford Loan Entrance Interview/Counseling Session - Required for first time Stafford Loan borrowers.
  2. Stafford Loan Exit Interview/Counseling Session - Required for students who either graduate, withdraw fully or are registered below half-time.
  3. Perkins Loan Entrance Interview/Counseling Session - Required for first-time Perkins Loan borrowers.
  4. Perkins Loan Exit Interview/Counseling Session - Required for students who either graduate, withdraw fully or are registered below half-time

F. Federal PLUS Loan

  1. PLUS Loan Pre-approval Request: Undergraduate | Graduate - (.pdf)
    "You may complete the PLUS pre-approval process with your lender by four different options: online, over the phone, via fax or by mail."
